First Name	one piece of valid and unexpired government-issued identification printed in English, in your legal name, which must have a clear photograph and your signature and one of the following: Nickname to Legal Name: your birth certificate Old Legal Name to a New Legal Name: official, legal documentation of the name change Examples of this documentation include: Certificate of naturalization Court order for a name change
Last Name	one piece of valid and unexpired government-issued identification printed in English, in your legal name, which must have a clear photograph and your signature and official, legal documentation of the name change Examples of this documentation include: Marriage certificate Divorce decree Certificate of naturalization Court order for a name change
Social Security Number	one piece of valid and unexpired government-issued identification printed in English, in your legal name, which must have a clear photograph and your signature and a copy of your Social Security card and a copy of your score report bearing the incorrect Social Security number, if available
